Job Description

Overview

Coordinates patient access from referral to office visit. Assists in the coordination and planning of multiple healthcare services. Maintains communication with patients and health care providers to optimize access and monitor patient satisfaction. Facilitates a seamless transition of care in collaboration with patients, physicians, and stakeholders involved in the care of the patient. Responsible for program coordination and development of the Gender Affirmation Surgery Program with referring providers, department and facility providers, and partner organizations, including Eskenazi and IU School of Medicine.

 High school diploma or equivalent required. Bachelor's degree preferred.
• 0-3 years of experience required.
• Knowledge of medical and social services required.
• Clinical certification/licensure per unit/department specialty preferred.
• Experience working as part of a multidisciplinary clinical team preferred.
