The Program Coordinator - Cardiovascular operates as a clinical expert, liaison and consultant for cardiovascular caregivers and families. This position is responsible for planning, coordination, integration, education and the ongoing evaluation of the delivery of care across the East Central region.

As a regional team member, the coordinator is responsible for assuring the delivery of coordinated, cardiovascular care in the pre-operative, peri-operative and post- operative settings. Facilitates a detailed transition of care throughout the continuum across the region in collaboration with patients, physicians, vendors and all stakeholders involved in the care of the Watchman/TVAR patient and is responsible for assuring the team adherence to the program standards established for Watchman and TVAR patients.

In addition to the above, the Program Coordinator drives care coordination by actively providing consultative screening and evaluation of patients to direct and enhance access to IU Health services and improve patient, family, and referring physician satisfaction. Serves as a link between a patient, their family, the referring physician, the care team and the IU Health specialists caring for them as well as to community resources.. Consults with patient navigators and other key staff on specialized care plans and inter-professional assessments as a liaison between staff, patients and physicians, across the region.

This position exists to assure and evaluate delivery of evidence based care according to national and statewide clinical practice guidelines; facilitate appropriate access across the East Central region; lead performance improvement initiatives; assist in developing clinical pathways; and assuring care management occurs in an efficient manner while improving patient safety and the overall customer experience. Assures standards of practice adhere to the program standards established the Service Line. Coordinates data collection with the Watchman and TVAR registry team for IU Health.

• BSN preferred.
• RN with 2-4 years of experience.
• Would like previous cardiovascular patient care experience. Able to discern data, data abstraction, patient education and has a working knowledge of software applications. Able to function independently and use critical thinking.
