IU Health's System Health Solutions has a wonderful opportunity for a Health Coach to join our Healthy Results division! This position is a hybrid remote position that requires daytime and evening hours. Three days of day shift and two evening shifts (10:30am - 7:00pm one day and 12:30pm - 9:00pm one day).
The Health Coach promotes team member well-being by increasing awareness of biometric results and resources, identifying and preventing chronic disease, and elevating participant's engagement in their own health and wellness. Engages risk stratified members by providing proactive outreach, telephonic health coaching sessions and onsite delivery of programs to individuals and groups. Utilizes behavior change theories, established protocols and collaboration with population health team members to support participants and create customized wellness plans. Partners with participants to set goals, resolve barriers, and achieve long term behavior change. Conducts group coaching as well as development and delivery of presentations as needed.
