Overview

The Community Health Specialist will engage with the community and at clinics. This role is expected to be out in the community providing services such as blood pressure screenings and social resources. They will be an integral piece in improving self-sufficiency, health equity, and overall community health. They serve as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.

Requirements: 

• High School Diploma or equivalent required
• 0-3 Years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
• Background in Medical terminology/medical preferred
• CNA/PCT license preferred
• Proficiency in Spanish preferred
