Overview
Indiana University Health Home Infusion Pharmacy is seeking a full-time pharmacist with the skills – both clinical and operational - required to provide excellent pharmaceutical care to patients in the home environment. Our pharmacists coordinate patient care with discharge planners, providers and home care nurses; assess appropriateness of therapy and monitor patient response; create care plans and document patient progress toward goals; intervene as clinically appropriate; and educate providers, nurses, patients and caregivers in support of safe and effective home infusion therapy. This position will oversee activities of technicians and other support staff, interpret and evaluate orders, recommend adjustments to ensure appropriateness for the home environment, and formulate, verify and dispense compounded sterile products, non-compounded ancillary prescriptions, and/or specialty drugs, ensuring timely distribution to patients throughout the state of Indiana. This is a dynamic work environment which requires flexibility to meet business needs and adaptability to frequent changes. Our pharmacists work primarily day shift, with on-call and minimal weekend/holiday coverage responsibility. The ideal candidate will be a strong team player with prior home infusion and/or sterile compounding experience, including familiarity with USPNF General Chapters 797 and 800.
This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.
