Job Description

Overview

Riley Hospital in downtown Indianapolis is currently seeking a Full-Time, Social Worker to join the Multidisciplinary Neuromuscular Team! This is a 5 year grant funded position. This position will primarily serve pediatric patients with neuromuscular disease and their families to promote the goal of comprehensive care delivery through a collaborative model of family-centered goal setting and communication through shared plans of care.  Working closely with nurse navigators, the social work position will assess families for social drivers of health and promote family empowerment and self-advocacy.  Care delivery is organized to include neurology, cardiology, endocrinology, and pulmonology services. 

The Social Worker will work closely with the Nurse Coordinator and Care Team to coordinate the care needs of assigned patients and develop comprehensive plans to manage care delivery across the patient care continuum. Partner with patients and their specialty physicians to develop customized care plans based on their individual needs and preferences. Advocates for the patient. Focus will be around addressing social determinants of health. Additionally, this position will collaborate with physicians, nurses, health professionals, community agencies, and others to ensure patients’ medical and social needs are met.

 Master's Degree in Social Work required.
• CPI (Non-Violent Crisis Intervention) certification required within 6 months.
• Permanent LSW licensure required, LCSW licensure preferred.
• 3-5 years experience preferred in one of the following areas (mental health, Department of Child and Family Services, adult protective services, Veteran's Administration, substance abuse).
• Requires experience working with geographically and financially diverse patient populations.
• Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling and crisis intervention to patients, families and staff.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
• Develop and maintain strong working relationships with the community, medical teams, law enforcement, APS/CPS, schools, probation and other agencies as indicated.
• Requires proficiency in Microsoft Excel and Word.
