We are looking to fill a Full-Time Hepatitis C Care Coordinator position (40 hours a week) to service the Bloomington/Columbus (Bloomington and Columbus Regional Hospital) area. Candidates will be required to work between 8:00am – 5:00pm, Monday – Friday. Some weekends as required.
The primary role of the Hepatitis C Care Coordinator will be to provide medical and nonmedical case management engaging high risk populations with medical care to receive treatment for Hepatitis C. They will serve as a bridge between the community and the health care, government and social service systems. They will assist individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. They will provide informal counseling, support, and follow-up to the community.
