Director- Value Based Care, Care Management

Job Description

Overview

The Director- Value Based Care, Care Management manages planning, directing and overall operation of the population health care management vision. The Director provides leadership to assigned personnel and ensures that all care management functions are performed accurately and efficiently, including managing ongoing analysis, development, and implementation of methods and systems to improve overall effectiveness of Care Management processes. The Director develops services, policies and procedures and exists to promote patient advocacy and quality patient care with unsurpassed clinical and cost outcomes. This position also partners with the Director of Population Health and VP of Population health as well as IU Health Plans executive leadership to create and execute overall population health strategy.


 Bachelor's Degree Required. Master's Preferred. Requires 7-10 years of relevant experience.
• Requires proficiency in interpreting and working with data.
• Requires experience with managing a budget and P&L.
• Experience leading teams within Population Health or Care Management preferred.
• Experience working within a matrixed organization and complex working environment preferred.
