Job Description

Overview

We are seeking a Clinical Pharmacist with Hematology Oncology experience for the Schwarz Cancer Center at IU Health North Hospital! This facility provides true multi-disciplinary care with many specialties and services.

The Schwarz Cancer Center is seeking a highly motivated Clinical Pharmacist with experience and/or training in hematology/oncology to expand our clinical pharmacy program. This position will work collaboratively with our oncology pharmacy team, focusing primarily on our outpatient medical oncology populations seen in the infusion center. The pharmacy department has a strong residency program and is affiliated with Purdue and Butler University Schools of Pharmacy and Indiana University School of Medicine. Responsibilities include: direct participation in medication management and education of patients, review of care plans and chemotherapy regimens, order set creation/development/revision, participation in IRB activities, precepting pharmacy residents/students, and collaborating with the medical staff on clinical programs and research.

The position exists to deliver comprehensive medication-related care for the purpose of The Pharmacist position at IU Health North Hospital in Carmel, Indiana exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.

 Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.
• Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be required.
