  • Servant Leadership that values, appreciates, supports and builds-up our Team!
  • Visit patients and families in IU Health hospitals or their homes which could be a single family dwelling, apartment or condo, assisted living facility or skilled nursing facility. This position allows you to spend quality one on one time with your patients at a time when it is most needed.
  • Assist and counsel hospice patients and families with advance directives, health related financial, social and emotional concerns
  • Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness
  • Participates in the development of the individualized plan of care, involving the patient and family.
  • Attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
  • Provide Bereavement support to family members up to 13 months after loved ones death.
  • Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.

Medical Specialty:

Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday

Education:

  • Master's (Required)
  • LSW required; LCSW preferred
  • Requires knowledge of state and county adult and child legal mandates

Experience:

  • Social work in a healthcare setting: 1 year (preferred)

 Master's Degree in Social Work required.
• CPI (Non-Violent Crisis Intervention) certification preferred.
• Permanent LSW licensure required, LCSW licensure preferred.
• Requires relevant health care, mental health or community-based experience.
• Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling, and crisis intervention to patients, families and staff.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
• Develop and maintain strong working relationships with the community, medical teams, law enforcement, APS/CPS, schools, probation and other agencies as indicated.
• Requires proficiency in Microsoft Excel and Word.
•0-3 years of Medical Social Work experience required.
