IU Health Physicians is currently seeking a Clinical Nurse Navigator, who is inspired by challenging and meaningful work for the good of every patient. This Clinical Nurse Navigator will support our hepatology pod through a specialized program aimed at reducing transfers, reducing readmissions and increasing patient access and care coordination across various IUH hospitals and other healthcare organizations. Position will be required to support several Transitional Care Clinics (TLC clinics) by going to clinic with our group of Hepatology APPs at IUH University Hospital and Spring mill office building (Carmel). This position works Monday-Friday and will offer a hybrid remote work environment, based on the clinic needs, upon the completion of training.

The Clinical Nurse Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. Partners with patients and their primary physicians to develop customized care plans based on their individual needs and preferences. Collaborates with physicians, nurses, health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion. Advocates for the patient. Balances care needs and financial considerations to ensure efficient and effective treatments are achieved.
