Overview

Description  

The Patient Transfer Center RN will evaluate, coordinate, process, screen, document patient transfers into the IU Health System, and collaborates with relevant business partners for appropriate level of care and transport of the patient into or out of the IU Health System. The incumbent will obtain appropriate medical and demographic information, and coordinate communication between the referring and receiving physicians as well as between healthcare facilities. The Patient Transfer Center RN will perform essential duties related to patient transfer/transport and will coordinate all components of patient care needs during the transfer/transport process. The Patient Transfer Center RN will efficiently engage appropriate departments in the transfer/transport processes, and will assign beds for all scheduled and unscheduled admissions including patient transfers/transports.

  1. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license with a minimum of 5 years as a Registered Nurse.
  2. Requires a minimum of 3 years critical care, emergency department, or flight nursing experience or equivalent.
  3. Case management/resource management experience is a plus.
  4. Requires exceptional communication, listening, and written skills.
  5. Requires strong interpersonal and organization skills emphasizing leadership, teamwork, collaboration, and attention to detail.
  6. Requires excellent critical thinking skills to work autonomously and multitask effectively.
  7. Requires the ability to problem solve and resolve conflicts effectively in the best interest of the patient and family.
  8. Requires computer experience. BSN preferred. Effective 02.13.11, any RN hired must have graduated from a nursing program accredited by the ACEN or CCNE. Case management experience a plus. Requires current State of Indiana nursing license.
  9. Requires Basic Life Support,(BLS), Advanced Life Support (ACLS), and Pediatric Advanced Life Support (PALs) certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. v2
  Responsibilities  

Continuous Improvement - Promotes and supports the participation of staff in applying the principle/model of continuous improvement to improve existing processes and enhance the effective utilization of resources (human and material).

HIPAA -  Maintains at all times the confidentiality of any patient or employee medical, financial, or personal information, and other information, records and data to which there is access. Views, uses or discloses such information only for reasons necessary to perform duties. Responsible to challenge unauthorized individuals from viewing such confidential patient or employee information or accessing restricted areas.

Clinical Judgment - Applies sound clinical judgment by utilizing current patient care standards, established protocols, Interqual, and past clinical critical care experience when assessing the needs of each patient. Collaborates with physician to identify the appropriate level of care and initiates time sensitive priority Level One patient program/procedures when appropriate. Collaborates with colleagues and clinical staff as needed acting in the best interest of the patient and family. Facilitates transfer of the patient into the IU Health System and collaborates with relevant business partners for appropriate level of transport of the patient into or out of the IU Health System. Identifies and problem solves system challenges that may impede timely transfer/transport process.

Communication - Utilizes various IT solutions (i.e. Teletracking, Cerner, Amcom) systems. Maintains Teletracking Board and verifies board accuracy per departmental policy for census count. Communicates often and effectively to customer base to coordinate patient transfers/transport. Continues to monitor the transfer/transport process throughout and communicates pertinent information to the referring/receiving physicians and healthcare team members until the transfer/transport process is completed. Collaborates with Lifeline Communications personnel as needed. Collaborates with Nursing and Patient Care Services to place patients in the appropriate bed assignment based upon established guidelines and patient needs.
