Job Description

Overview

What are the hours? 8am-4:30pm **Must be able to flex 25% of time to cover community events taking place during nontraditional work hours (evenings, weekends).

Is this an in-office, remote or hybrid position? Community Health Specialists supporting the Hypertension/Social Determinants of Health program will spend 2-3 days working in a clinic setting and must have reliable transportation to conduct community visits. 1-2 days per week can be remote as long as Community Health Specialists is meeting goals and up to date on assignments. Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 25% of time). 

Would this role require Travel for in-person meetings with individuals most of the time or would most meetings be phone calls? Most of the outreach to patients occurs telephonically and there are times when Community Health Specialists connect in-person with patients to deliver items and provide education. Community Health Specialists are also required to participate in team meetings/retreats, which may be in-person.

Is there travel to individual’s homes? Yes, but will be paid for mileage.

Bilingual a Plus

 High School Diploma or equivalent required.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
