The Clinical Nurse Ambulatory Program Coordinator provides nursing, administrative and functional guidance to staff of assigned program. May provides assistance relating to operational and business functions; manages projects and acts as a liaison to other departments. As clinical expert, liaison and consultant for patients, this position is responsible for planning, coordination, integration, education and the ongoing evaluation of the delivery of care to patients. Serves as a link between a patient, their family, the referring physician, the care team and the IU Health specialists caring for them as well as to community resources. Consults with patient navigators and other key staff on specialized care plans and multidisciplinary assessments as a liaison between staff, patients and physicians. This position exists to assure and evaluate delivery of evidence based care according to national and statewide clinical practice guidelines; facilitate appropriate access; lead performance improvement initiatives; assist in developing clinical pathways; and assuring care management occurs in a cost effective manner while enhancing patient safety and the overall customer experience. Assures standards of practice adhere to the program standards established the Service Line.
