We are looking for a full-time days Social Worker to join our Virtual Integrated Social Work team.

Integrated Social Work (ISW) offers both virtual and in-person, urgent and non-urgent social support throughout IU Health primary care and select specialty clinics. The ISW team works as an extension to the patient’s outpatient care team to address social determinants of health and work to mitigate barriers to health care. Assess patients' and families' psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness, and ability to cope. Counsels patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs. Completes psychosocial assessments of patients and facilitates referrals to appropriate service providers. Serves as the point person in child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence. Serves as liaison between Indiana University Health and state and county child and adult protective teams.
This position will be primarily remote with the possibility of in office days for trainings and team meetings.
