Overview
IU Health Hospice provides patients with comprehensive care delivered by a highly trained, interdisciplinary team led by the hospice medical director. The IU Health Hospice Social Worker supports patients, families, and the health care team to ensure the best possible experience. Patients can receive hospice care in a home setting. This may be a private residence, an assisted living facility, a skilled nursing facility, or a group home. IU Health Hospice also provides General Inpatient (GIP) hospice care in our hospitals [and in our Hospice House BLOOMINGTON ONLY].
They will work to effectively communicate and collaborate with all members of the health care team. This position helps to ensure that care is delivered in a safe and timely manner, and the patient and family are provided with all necessary resources and education. The MSW plays an integral role in interpreting and communicating around complex patient/family needs related to the patient’s terminal diagnosis.
Our Hospice MSW assesses patient and family psychosocial risk factors through evaluation of support systems and ability to cope. They provide counseling for patients and families regarding the emotional, social, and financial impacts of terminal illness. They serve as a point person in safety, abuse, neglect, and exploitation situations.
