The ISR Spine Navigator will play a key role in enhancing patient experience, improving surgical outcomes, and coordinating care for patients undergoing spine surgery.  The spine navigator engages patients and their families throughout the episode of care by lending support, providing patient education, and setting expectations for the pre- and post-surgical processes.  Navigators also work with the Spine Service Line leadership team to assist with program growth strategies, optimization of care, implementation of evidenced-based care, quality and cost management, patient education programs, physician recruitment, and physician engagement. 

Responsibilities

  • This navigator will serve as spine content expert / guru for the multidisciplinary entities in the hospital and post hospital setting (i.e. home health) for identified gaps and need for improved processes.
  • Driving change and implementing best evidence practices per professional organizations and research.
  • Collaborating with marketing and physician liaisons to enhance program visibility for referring providers. This navigator will help develop community outreach initiatives and education.
  • Responsible for working with clinical nursing specialists and NPD (nursing practice development) team to coordinate ongoing education of staff who provide care for spine patients.
  • Collaborating with system teams on developing and revising order sets.
  • Participating with physician interviews and provider onboarding.
  • Supporting Spine registry work
  • Responsible for collecting trends and analyzing quality metrics and customer service outcomes.
    • Applies data analysis skills to evaluate /implement /facilitate best practices across the care continuum.
    • Organizes program data for spine program; identifies focuses points.
    • Communicates patient satisfaction scores and barriers to leadership.
  • Helping develop program strategy for Centers of Excellence such as Anthem Blue Distinction
    • Prepares for onsite and virtual surveys.
    • Collects, analyzes, and maintains program outcome metrics.
    • Collaborates with AHC care management team on submission.
  • Participates in Spine Steering Committee with surgeon co-lead(s).

The RN Nurse Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. Partners with patients and their primary physicians to develop customized care plans based on their individual needs and preferences. Collaborates with physicians, nurses, health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion. Advocates for the patient. Balances care needs and financial considerations to ensure efficient and effective treatments are achieved.

  • Requires an Associates of Nursing (ASN).
  • Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
  • Requires Basic Life Support certification through the AHA.
  • Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
  • Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
  • Requires knowledge of personal computers using various types of word processing and database software.
  • Requires clinical work experience, medical terminology.
  • Requires understanding of medical record requirements, regulations and policies.
