This role provide project management for the prioritized workstreams as identified by the Director and Steering Committee. Manages a complex portfolio of projects for Social Determinants of Health within Community Health across the state. Ensures projects are managed appropriately, utilizing standard project management methodologies, processes, tools, and resource planning. Responsible for achieving specific project metrics and deliverables within a specified timeline across multiple regions. Develops project plans based on customer specifications that result in optimized performance, cost savings, efficiency and overall return on investments. Collaborates across teams to ensure alignment and delivery of end results.
