The Post Acute Liaison Case Management Manager partners with the Director of Care Management in developing and implementing post-acute case management services. Leads case management team to achieve system-wide population health goals. Delivers ongoing coaching, mentoring, and support to direct reports. Resolves issues and addresses staff, patient, and provider concerns. Manages ongoing analysis, development, and implementation of methods and systems to improve overall effectiveness of case management processes. Engages team to provide quality patient care with unsurpassed clinical and cost outcomes. This role is focused on coordinating post-acute care, specifically in the Skilled Nursing Facility environment. The role manages ongoing analysis, development and implementation of methods and systems to improve overall effectiveness of SNF-focused case management processes for value-based care patients, including the successful transition from the SNF to home. This role engages, cultivates, and strengthens relationships with SNF staff and leaders to provide quality patient care.
