Job Description

Overview

This position will be apart of the growing Pediatric Integrated Behavioral Health team and will work within pediatric and family medicine practices alongside the primary care teams to provide evidence-based behavioral health interventions to children, adolescents, and/or their caregivers. 

Cares for patients who have complex psychosocial needs or require assistance from social support programs or community assistance. Interviews patients and relatives to obtain social history relevant to medical problems and planning. Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care. Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies. Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization. This position will serve IU Health Jay, Albany, and Liberty Corner.  

 

****LBSW temp or permanent licensure required, permanent license should be obtained withing 6 months of hire. 

 

 0-3 years of experience required. Bachelor's Degree in Social Work required.
• Permanent LSW licensure preferred.
• Requires strong clinical assessment, planning and goal development skills.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Requires strong counseling skills to help patients and families understand, accept and follow medical recommendations.
• Requires proficiency in Microsoft Excel and Word.
