This RN Ambulatory Supervisor will collaborate with leadership to address the needs of the Community Health Program in Indianapolis, IN. This role will require travel to other regions in the state. The RN Ambulatory Supervisor will have the opportunity to lead a team of 5+ Community Health Workers spanning across 4 regions. This position will work day shift, with occasional nights and weekends. The RN Ambulatory Supervisor facilitates and supports the local adoption of care redesign strategies. Collaborates with IU Health system leaders to develop and execute on regional performance improvement strategies that are aligned with IU Health system goals. Assists in the development of strategic plans for the system and leads the development of sustainable, documented processes to support the business. Executes on initiatives based on sound data and leads performance improvement initiatives. Guides and supports the local implementation of care redesign methodologies. Collaborates in the development of clinically based performance analytic tools. Reports progress and clinical performance improvement opportunities to key stakeholders. Specific to the Community Health Program, additional responsibilities include, serving as a clinical subject matter expert. The incumbent will play a critical role in advancing strategies clinical priorities in the area cardiovascular disease, hypertension. Provide clinical consultation to community based CHWs as needed for various patient cases related to hypertension management. Support performance improvement for CHWs, including workflow documentation and audits, relationship building with clinic managers, etc. Ensure CHWs are consistently delivering excellent customer service and capturing patient impact stories. Track, report, and improve operational metrics. Manage financial and operational grant obligations. Maintain and revise policies and standard work for CHWs embedded in the community settings. Manage personnel, including timekeeping activities, performance management, team engagement, recruitment, and onboarding. Coordinate BP cuff orders, distribution, and tracking for all regions. Manage centralized CHW email address and voicemail. Provide support to program administrators as needed for various quality improvement projects.
