IU Health is seeking a dynamic and collaborative leader to join our team as an Ambulatory Value-Based Care (VBC) Manager. As a key member of our leadership team, you will have the opportunity to work closely with system-wide leaders to drive meaningful change and improvement in patient care.

As the leader with approximately 20 direct reports, you will be responsible for guiding and supporting your team to deliver high-quality, patient-centered care to achieve exceptional clinical and cost outcomes. This position will work a hybrid model schedule.

Partners with the Director of Care Management in developing and implementing care management services within a defined IU Health region. Leads care management team to achieve system-wide population health goals. Delivers ongoing coaching, mentoring, and support to direct reports. Resolves issues and addresses staff, patient, and provider concerns. Manages ongoing analysis, development, and implementation of methods and systems to improve overall effectiveness of Care Management processes. Engages team to provide quality patient care with unsurpassed clinical and cost outcomes.

• Bachelors Degree in Nursing  or LCSW required.
• Masters Degree in Nursing preferred.
• 5-7 years of relevant experience required.
• Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license or LCSW.
• Certification in Care Management preferred.
• Health Plan Case Management Leadership or Population Health experience strongly preferred
