Job Description

Overview

Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. 

Community Health Specialists supporting the Hypertension program will spend 2-3 days onsite supporting various workflows, including patient outreach to help improve primary care access. 

Community Health Specialist must have reliable transportation to conduct home visits, with mileage reimbursement. 1-2 days per week can be remote as long as Community Health Specialists is meeting goals and up to date on assignments. Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 10% of time).  Most of the outreach to patients occurs telephonically and there are times when Community Health Specialists connect in-person with patients to deliver items and provide education. Coaches usually drive to 1-2 locations each day when they're not remote. Community Health Specialists are also required to participate in team meetings/retreats, which may be in-person.  Training and education will be provided as part of the orientation process. 

 High School Diploma or equivalent required.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
