New Graduates
New registered nurse (RN) graduates benefit from a comprehensive, personalized support system. Responsible for serving as a Nurse in the Remote Patient Monitoring program (RPM). Monitors members using established disease specific protocols. Provides clinical support for members on program via telephonic outreach and inbound calls. The Nurse will work collaboratively with the multidisciplinary healthcare team to provide the best outcomes for the patient. This position requires strong critical thinking skills, ability to multi-task, be a great teammate, with effective/ good communication skills. We are looking for a fast learner, with a strong work ethic, and ability to work autonomously. Strong Interests in technology and innovation preferred. Previous telephonic case management experience preferred, but not required.
In this role, the RN will support and educate patients with chronic conditions such as diabetes, CHF, COPD/asthma, heart disease, anxiety or depression. You will be responsible for setting goals and conducting transitional care management activities with high risk patients discharge from the hospital.
Schedule: Three 12-hour shifts per week, 7a-7p, with a weekend rotation. This is a HYBRID position: 80% in community/patient homes, 20% remote.
Our Home Health RN's are offered competitive compensation, consistent with all RN positions across the IU Health system, including shift differentials.
