Overview
Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.
Community Health Specialists supporting the Infant/Maternal Health program will spend 2-3 days working in a clinic setting and must have reliable transportation to conduct home visits, with mileage reimbursement. 1-2 days per week can be remote as long as Community Health Specialists is meeting goals and up to date on assignments. Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 10% of time). Most of the outreach to patients occurs telephonically but Community Health Specialists regularly connect in-person with patients to deliver items and provide education. Coaches usually drive to 1-2 locations each day when they're not remote. Community Health Specialists are also required to participate in team meetings/retreats, which may be in-person. Training and education will be provided as part of the orientation process.
8:00 am - 4:30 pm Must be able to flex 10% of time to cover community events taking place during nontraditional work hours (After 5 pm on weekdays or Saturday/Sunday).
