The Pulmonary Disease Care Navigator is responsible for facilitating and managing the respiratory care coordination of the acute and chronic lung disease population. This role evaluates patients admitted with a diagnosis of chronic lung disease for inclusion criteria to determine the need for disease-specific education and follow-up to reduce hospital admissions and readmissions. This role provides education, materials, and/or referrals appropriate for the patient’s educational level and specific needs while maintaining accurate documentation and data for reporting purposes.
