Summary
IU Health Hospice Care provides patients with comprehensive care delivered by a highly trained, interdisciplinary team led by the hospice medical director. IU Health Hospice LPNs work closely with RN case managers to provide skilled care in a patient’s home setting. This may be a private residence, an assisted living facility, a skilled nursing facility, or a group home. The holistic services that our Hospice professionals offer add to our patients’ overall quality of life.
Our Hospice nurses participate in a team approach to serving patients and have flexibility to provide patients with individualized care and to support patients’ family members. Examples of the care you will provide include infusions, pain pumps, line care, catheter care, wound care, chronic disease process management, patient / family education, and compassionate end of life care.

Schedule
Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.
