Indiana University Health Adult Academic Health Center (IUH AAHC), Department of Pharmacy is recruiting a clinical pharmacy specialist to join our inpatient internal medicine/surgery abdominal solid organ transplant team. The team member will provide comprehensive clinical care to patients with severe or complex illnesses. The successful candidate is highly motivated, resourceful, and has outstanding leadership and communication skills. 

Methodist and University Hospitals, as part of the AAHC, provide nationally recognized clinical care to patients from across Indiana and beyond. Our pharmacy practice model includes a multi-layered approach to supporting our patients and healthcare staff, by combining decentralized clinical generalists, clinical specialists, operations specialists and technicians to meet patients’ needs in a comprehensive manner. Pharmacists are an integral aspect of patient care at IU Health, with a strong presence and excellent relationships with providers and nursing partners across all specialties. The ideal candidate has experience providing pharmacist services and advancing practice in the setting of adult solid organ transplant. medical and/or post-operative care. 

Responsibilities: 

  • Providing clinical decentralized pharmacy services for solid organ transplant patients in an inpatient and ambulatory setting in internal medicine setting 
  • Monitoring and intervening drug therapy to improve patient outcomes
  • Addressing dosing consults (antibiotics, anticoagulants, immunosuppressants etc.) including adjustments and documentation
  • Precepting student pharmacists and PGY1/PGY2 pharmacy residents
  • Providing leadership for implementation of drug policy guidelines, protocols, and clinical decision support
  • Developing strategies to continuously improve the quality and provision of pharmacy services
  • Ensuring compliance with quality and regulatory standards
  • Collaborating on clinical research projects and studies
  • Educating providers on evidence-based medication utilization, effectiveness, dosing, and adherence to improve patient outcomes
  • Participating in daily, multi-disciplinary, clinical rounds on dedicated unit for collaborative medication recommendations

Qualifications:

  • Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.
  • Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be required.
  • Competency of transplant pharmacotherapy required.
  • Board certification in pharmacotherapy or solid organ transplant, preferred
