Job Details

We are seeking an RN with 3-5 years of oncology experience! Outpatient Experience is preferred.

Responsible for coordination of the care provided to the Infusion Oncology Therapies patient. Ensures access to appointments, support programs and education, working with the community to enhance coordination to the patient and caregivers, assisting the providers and community based specialists in providing optimal care and providing the patient, family and caregivers with the support and knowledge they need. The Patient Navigator fulfills the primary responsibilities for coordinating a multi-disciplinary approach in the diagnostic, evaluation, treatment and facilitation of the care process throughout the care continuum up until end of life, survivorship and in support of long-term surveillance. This position also exhibits skills as an empathetic patient advocate and skilled communicator and serves as a representative for the treatment programs. By working with all applicable departments, this position will ensure that all patient education and coordination be consistent and aligned with and/or an extension of the providers' recommendations.

This position has Comprehensive management of all patients receiving oral chemotherapy which includes prescription acquisition, financial assistance assessment, medication education, toxicity management and management of monitoring parameters, along with a coordination of care for concurrent treatments.

QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES
  • ASN required; BSN preferred.
  • Requires current state of Indiana licensure as a Registered Nurse.
  • Basic Life Support certification required.
  • 3-5 years of nursing and/or clinical care coordination (case management).
  • Must achieve a nurse navigator certification within 3 years of date of hire.
  • Experience in oncology (Radiation or Medical) preferred.
  • Oncology Certified Nurse (OCN) preferred; certification required within 2 years.
  • Chemotherapy/Biotherapy Certified (ONS) preferred.
