Job Description

Overview

Provides patient education on applicable disease to identified and/or risk stratified target audience. Educates patients about disease risk factors and prevention, disease state, role of medications, and health counseling related to management of chronic diseases. Assesses, implements, evaluates, and follows up on enrolled participants. Identifies gaps in care and communicate and/or navigate to appropriate referrals. Conducts proactive outreach and intervention with risk stratified individuals.

Location: Currently remote with possible combination of on-site in the future.

Shift: Rotating

Job Duties Include:

  • Utilize adult education techniques to educate patients about disease risk factors and prevention, disease state, role of medications, and health counseling related to management of chronic diseases.
  • Assess, Implement, Evaluate, and conduct Follow-up on all participants enrolled in Condition Management program. Identify gaps in care and communicate/navigate to appropriate referrals.
  • Conduct proactive outreach and intervention with risk stratified individuals.
  • Act as a resource person to the patient and other health care professionals within Healthy Results.
  • Preferred experience:
  • 3-5 years of patient education experience.
  • knowledge of common medical conditions and disease states.

Success is accomplished by: conducting educational sessions via one-on-one, group setting, workshops and telephonically; following National guidelines on standards of care; participating in cross-training with the health coaching team; and updating patient education materials and protocols.

Requirements 

Bachelor's degree required.
• Active Indiana RN License required.
• Requires 5-7 years of clinical or relevant experience.
• Certified Diabetes Educator preferred.
• Certified Diabetes Educator preferred.
• Requires proficiency with Microsoft applications.
• Minimal travel within the state required.
