Overview
The Director – Population Health Care Management will lead the planning, directing and overall operations of the population health care management vision. They will provide leadership to assigned personnel and ensure that all care management functions are performed accurately and efficiently. This leader will manage the ongoing analysis, development and implementation of methods and systems to improve overall effectiveness of Care Management processes. This Director will have a unique opportunity to impact the organization in a large way as population health is a part of the IU Health 5-year strategic plan.
Other Opportunities Include:
· Developing services, policies and procedures and exists to promote patient advocacy and quality patient care with unsurpassed clinical and cost outcomes
· Working with key leaders to create and execute overall population health strategy
Qualifications/knowledge/skills/abilities
· Bachelors degree required. Masters preferred.
· 7+ years of relevant experience preferred with majority or all that time being in a leadership role within a matrixed organization and complex working environment.
· Experience leading teams within Population Health or Care Management strongly preferred.
· Proficiency in interpreting and working with data.
· Experience managing a budget and P&L.
· Strong relationship and leadership skills.

Reports to: Executive Director – Population Health
Direct reports: 5 manager level, total span of control of 95
Other Key Relationships: IU Health Plans President and other leadership, IU Health Physician Groups, Regional Hospital leadership, Inpatient Care Management Teams and various system leadership
