Job Description

Overview

Duties for the Nurse Navigator at the Schwarz Cancer Center include, but are not limited to:  screening and assignment to physician; data gathering (records, images, pathology); scheduling appointments; information to patient (new patient packet, preparation for visit); tumor board referral; database entry; initiation of prior authorization for treatment plan (XRT, Systemic Parenteral Therapy); coordination with specialty pharmacy/prior authorization for oral anti-neoplastics; monitor oral anti-neoplastic therapy adherence, toxicity; track outside studies (labs, scans); contribute to Survivorship Plan; communicate with outside physicians (referring, primary care); physician directed interventions (ie, follow up with tumor board); coordinate with inpatient team for hospitalized and discharging patients; and FMLA/disability paperwork.

Responsible for coordination of the care provided to the patient. Ensures access to appointments, support programs and education, working with the community to enhance coordination to the patient and caregivers, assisting the providers and community based specialists in providing optimal care and providing the patient, family and caregivers with the support and knowledge they need. The Patient Navigator fulfills the primary responsibilities for coordinating a multi-disciplinary approach in the diagnostic, evaluation, treatment and facilitation of the care process throughout the care continuum up until end of life, survivorship and in support of long-term surveillance. This position also exhibits skills as an empathetic patient advocate and skilled communicator and serves as a representative for the treatment programs. By working with all applicable departments, this position will ensure that all patient education and coordination be consistent and aligned with and/or an extension of the providers' recommendations.

The salaried Nurse Navigator will work Monday-Friday, 8 am – 5 pm with no weekends, holidays or on-call.   This position will provide navigation services for Multiple Myeloma patients.  Services include:  IU School of Medicine research, adolescent and young adult program, genetics, high risk screening, integrated health, laboratory, nutrition, palliative care, PET/CT, pharmacy, precision genomics, psychology, radiation oncology, same-day clinic and social work.

·        Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.

·        Requires current state of Indiana license as Registered Nurse.  Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.

·        Experience in multiple myeloma preferred.

·        Oncology Certified Nurse (OCN) preferred.

·        Chemotherapy/Biotherapy Certified (ONS) preferred.

·        Requires Basic Life Support certification through the AHA.

·        Other advanced life support certifications may be required per unit/department specialty according to patient care policies.

·        Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.

·        Requires knowledge of personal computers using various types of word processing and database software.

·        Requires clinical work experience, medical terminology.

·        Requires understanding of medical record requirements, regulations and policies.
