Team Lead-Population Health Care Management - Lafayette, Indiana

Job Description

Overview

Serves as a liaison to the Manager of Population Health Care Management by overseeing the day-to-day care management activities and providing functional guidance to Care Managers. Resolves issues, conducts ongoing performance conversations with team members, and improves overall outcomes. Participates in the delivery of Care Management services through working collaboratively with the multidisciplinary team. Engages identified patients in appropriate care management programs and provides patient centered interventions. Supports the achievement of system-wide population health goals within the framework of continuous improvement methodologies.

 Associate's Degree in Nursing required.
• Requires current state of Indiana license as a Registered Nurse.
• Certification in Care Management preferred.
• Strong professional level knowledge of comprehensive clinical assessment skills preferred.
• Knowledge of Medicare/Medicaid, insurance and regulatory guidelines is preferred.
•3-5 years of relevant experience required.
• Minimum of 1 year experience in a management or lead role preferred.
