Job Description

Overview

The Behavioral Health Coach engages patients and encourages them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt life-long healthy behaviors. The Behavioral Health Coach helps patients unlock personal motivation to make lasting changes and simultaneously serve as the patients’ primary conduit to the health care provider as they help coordinate necessary services both within and outside of the health system. Ideal candidates will be knowledgeable of or have some understanding of cognitive behavioral approaches, short-term therapy approaches, anxiety, and depression.

Additionally, this role promotes well-being by increasing awareness of biometric results and resources, identifying and preventing chronic disease, and elevating participant's engagement in their own health and wellness. Engages risk stratified members by providing health coaching sessions and onsite delivery of programs. Utilizes established protocols and collaboration with population health team members to support participants and create customized wellness plans.

Location: Primary Care offices in Lafayette, West Lafayette, and virtual!

Preferred experience:  Healthcare/Behavioral Health, multidisciplinary team experience

 

Hours: 8:30-5, M-F. Some flexibility in these hours to meet the needs of PCP offices. Travel may be required to provide services to multiple clinic. Virtual care may be required in addition to face to face care.

Qualifications:

MSW preferred; or Bachelor's degree in Nursing, Health Promotion, Health Education, or a health-related field. 
• Health/Wellness Coaching Certification strongly preferred.
• Requires proficiency with Microsoft applications.
• Travel within the state required.
