Job Description

Overview

The Clinical Nurse Ambulatory Program Coordinator will provide nursing, administrative and functional guidance to staff for our new program, Hospital at Home.   This is an innovative care model that provides safe, high quality, hospital level care to older adults in their own homes.  The current hours for this position are Monday-Friday from 8 am – 5 pm, but flexibility is necessary as hours could change/shift into evenings as the program grows and needs are reassessed.  The Coordinator will identify patients in the Emergency Departments and Observation Units that could be cared for in their home setting, preventing a hospitalization.  This program will currently serve our West (Avon), Tipton, Saxony (Fishers, and North (Carmel) hospitals. One avoided hospitalization has the potential of saving the health system $15,000.  The Coordinator will work with remote patient monitoring teams, home health and population health leaders, and physicians in the ED and virtual care team.  Coordinator role arranges discharge from the ED with a Hospital at Home admission with coordinating needed medical equipment and other discharge needs.  Hospital at Home will focus on patients that are part of Population Health / IU Health Plans with these diagnoses:  CHF, COPD, pneumonia, cellulitis, UTI or an infection.  As the program develops, hours may be adjusted based on demand. Candidate must be flexible to working evening hours and weekends.

  • Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred. 
  • Requires current state of Indiana license as Registered Nurse.
  • Requires that the RN has graduated from a nationally accredited nursing program. 
  • Requires Basic Life Support certification through the AHA. 
  • Other advanced life support certifications may be required per unit/department specialty according to patient care policies. 
  • May require specialty certification.
