Job Description

Overview

The Manager of Care Management partners with the Director of Care Management in developing and implementing care management services within the Southern Indiana Physicians IU Health region and to lead the RN care management team to achieve system-wide population health goals. Delivers ongoing coaching, mentoring, and support to direct reports. Resolves issues and addresses staff, patient, and provider concerns. Manages ongoing analysis, development, and implementation of methods and systems to improve overall effectiveness of Care Management processes. Engages team to provide quality patient care with unsurpassed clinical and cost outcomes.


 Bachelors Degree in Nursing (BSN) required.
• 5-7 years of relevant experience required.
• Requires current state of Indiana license as a Registered Nurse.
• Certification in Care Management preferred.
