We are seeking an innovative and collaborative Paramedic for our Connected Care and Home Health care team!
This is an opportunity to continue utilizing your professional experience and skills as part of IU Health’s new and innovative in-the-home patient care models – the perfect opportunity for expanding expertise and collaborating with team members on the forefront of community-oriented health delivery.
The Paramedic will support the IU Health Connected Care Primary Care at Home, Home Health, and Adult Home Visit programs by delivering direct patient care in the home across Indianapolis and surrounding communities. These new models of care allow IU Health to meet our complex, chronically ill, or homebound patients where they are and however they are.
This team member will collaborate with specialized primary care and home health teams to engage patients in their home, provide direct patient care, identify potential home safety risks or other non-traditional barriers to health and wellness, and work with patients and providers to deliver comprehensive primary care services in the home with the assistance of telehealth technology. Paramedic will complete and document head-to-toe assessments, medication histories, venipuncture and capillary draws, administer vaccines, and so much more. This team member will also acquire consents and authorizations from the patient and setup necessary virtual visit video and audio equipment.
The right professional for this role will appreciate the purpose of these innovative care delivery models, collaborate with a diverse team of providers and staff to coordinate services, demonstrate excellence in patient engagement and care, and have the compassion to treat all patients with respect, empathy, and kindness.
