BASIC FUNCTION:

The Social Worker, also referred to as the Behavioral Health Clinician (BHC) partners with the primary care providers (PCPs) (including physicians, resident physicians, nurse practitioners, and physician assistants) in a consultative role to assess the behavioral health needs of primary care patients and provide brief evidence-based triage, behavioral interventions, and referrals. BHCs may also be asked to provide a range of services including screening for common conditions, performing assessments, and providing interventions related to chronic disease management programs. BHCs may be asked to collaborate to design and administer educational and therapeutic groups and act as liaisons to the community.  This position reports to the Psychologist assigned to the Primary Care clinic where the BHC is located. The assigned Psychologist will provide training, clinical supervision, and oversight of BHCs.

 

ESSENTIAL FUNCTIONS:

All employees will be held accountable to the Indiana University Health Standard of Service that is defined by Service Excellence

 

Accountabilities:

Maintains a visible presence to the PCPs during clinic operating hours. BHCs will be available for informal consultation by being in the clinic or available by phone. BHCs are available for same day and scheduled initial consultations with patients referred by PCPs.

Triages referrals from PCPs. BHCs will review and triage referrals from PCPs, determining which are or are not appropriate for the program. Those that are appropriate will be forwarded to office staff for scheduling. Those not appropriate will be returned to the PCP with explanation as to why they are inappropriate for our program and do not qualify. In addition to providing ongoing training regarding appropriate referrals, BHCs will help facilitate community referrals for those patients who do not qualify.

Performs initial evaluations and develops the treatment plan. BHCs will provide initial clinical evaluation of select patients, using the Biopsychosocial model of assessment, in order to assist PCPs with the development of a behavioral health-informed treatment plan. This plan may or may not involve formal or direct involvement from the behavioral health team.

Provides therapeutic interventions. BHCs will provide brief (up to 12 sessions), evidence based individual and/or family psychotherapeutic interventions. Interventions may include, but are not limited to, psychoeducation, Cognitive Behavioral Therapy, Motivational Interviewing for mental health or health behavior change, Interpersonal therapy, Acceptance and Commitment therapy, Metacognitive therapies, etc.

Maintains records. BHCs will maintain clinical records and other necessary paperwork in a timely manner to comply with all administrative regulations. BHCs will be required to account for the use of their time and responsible for completing documentation necessary for billing for services, as necessary. They also will communicate with PCPs regularly via personal discussion and/or brief consultant notes for the medical chart that explain assessment findings, interventions delivered, and recommendations made to the PCP.

Performs other duties that may be assigned from time-to-time by supervising Psychologist.

 

Productivity/Standards:

Meets productivity requirements as defined by hours of employment. Participates in the implementation of practice guidelines and standards of care. Collaborates with the primary care team to identify patients and populations who would benefit from behavioral intervention and education.

 

Coordination of Care/Resource Utilization:

 Promotes quality patient care outcomes and facilitates resource management. Monitors patient for follow through and coordination of care, minimizing fragmentation, and ensuring care linkage.

 

Teamwork/Collaboration:

Serves on intra- and inter-departmental committees, as requested, to discuss issues related to behavioral health services. Participates in referral development and marketing activities. Shares clinical observations and expertise with other clinical team members during formal and informal clinical case staffing. Serves as liaison with primary care providers and network providers.

 

Regulatory Compliance:

Adheres to legal and ethical standards as set forth by the clinician’s professional organization or affiliation and the organization’s policies and procedures. Performs and monitors quality assurance and peer review functions in keeping with established department protocols.

 

HIPAA:

Maintains at all times the confidentiality of any patient or employee medical, financial, or personal information, and other information, records, and data to which there is access. Views, uses, or discloses such information only for reasons necessary to perform duties. Challenges unauthorized individuals from viewing such confidential patient or employee information or accessing restricted areas.

 

Service Excellence:

Meets IUH expectations of preeminent service, including behavior that models Service Excellence--Every Encounter, Every Time, No Exceptions, No Excuses. Consistently demonstrates the four Service Commitments of the organization (Communication, Quality, Professionalism, and Environment).


 Master's Degree in Social Work required.
• CPI (Non-Violent Crisis Intervention) certification required within 6 months.
• Permanent LSW licensure required, LCSW licensure preferred.
•3-5 years experience preferred in one of the following areas (mental health, Department of Child and Family Services, adult protective services, Veteran's Administration, substance abuse).
• Requires experience working with geographically and financially diverse patient populations.
• Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling and crisis intervention to patients, families and staff.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
• Develop and maintain strong working relationships with the community, medical teams, law enforcement, APS/CPS, schools, probation and other agencies as indicated.
• Requires proficiency in Microsoft Excel and Word.
