IU Health Physicians Cardiology is looking for a Social Worker to work in our growing Adult Congenital Heart Disease (ACHD) program.

In collaboration with Riley Children’s Health, this program provides patients with congenital heart disease a seamless continuation of care experience to help them manage their heart conditions.  Our program is built on a foundation of lifelong multidisciplinary care that includes over 50 years of successful congenital heart surgery. We offer the only comprehensive statewide program with a group of cardiologists trained and board- certified in adult congenital heart disease. Close collaboration between highly skilled adult and pediatric cardiologists at IU Health and Riley at IU Health gives adults with congenital heart disease access to a wide range of advanced treatments and procedures. These options are only available through a program that combines dedicated experts and resources together in one system.

This team offers focused programs covering:
- maternal fetal medicine and cardio-obstetrics
- advanced cardiac imaging
- cardiovascular genetics
- multi-specialist care following the Fontan procedure
- pulmonary hypertension
- heart rhythm specialists managing complex arrhythmias
- innovative congenital heart surgery and transcatheter interventions
- heart failure, ventricular assist device, and heart transplant

The Social Worker effectively communicates and collaborates with all members of the health care team to facilitate the episode of care so the patient receives services in a safe and timely manner, and the patient and family are provided all necessary resources and education. Interprets and communicates complex patient/family needs and role models caring practices to other caregivers. Assess patients' and families' psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness, and ability to cope. Counsels patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs. Provides mental health assessments of patients upon physician referral. Provides therapeutic intervention to facilitate complex discharge planning. Serves as the point person in child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence. Serves as liaison between Indiana University Health and state and county child and adult protective teams. Provides intervention in guardianship (temporary/permanent), foster care, adoption, and mental health level II placements. Provides specialized mental health services including, but not limited to, Crisis Intervention and Addiction Assessments and recommendations for psychiatric referral or community resources. Assists with dangerous and potentially dangerous situations and takes a leadership stance to provide clinical intervention. Supports those undergoing acute crisis which often results in the use of de-escalation techniques necessary to avoid episodes of physical violence. Utilizes effective principles of interviewing alleged victims, perpetrators, family members, and significant others. Provides intervention for end-of-life, guardianship (temporary/permanent), foster care, adoption, etc.

 Master's Degree in Social Work required.
• CPI (Non-Violent Crisis Intervention) certification required within 6 months.
• Permanent LSW licensure required, LCSW licensure preferred.
• 3-5 years experience preferred in one of the following areas (mental health, Department of Child and Family Services, adult protective services, Veteran's Administration, substance abuse).
• Requires experience working with geographically and financially diverse patient populations.
• Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling and crisis intervention to patients, families and staff.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
• Develop and maintain strong working relationships with the community, medical teams, law enforcement, APS/CPS, schools, probation and other agencies as indicated.
• Requires proficiency in Microsoft Excel and Word.
