Job Description

Overview

The IU Health Population Health Physician Executive Director collaborates with the IUH Physician enterprise to establish and execute the clinical strategy by which the Population Health organization will improve the health of its membership while fostering appropriate and medically necessary care. Leads the deployment of innovative Population Health programs focused on improving the value of healthcare. Specific areas of focus will include Quality, HCCs and care management. This position reports to the Senior Vice President of Population Health and will work closely with the Executive Director of Population Health, IU Health Plans leadership and the leadership of the IU Health Physician enterprise. Creates value by helping to drive increased quality of care and decrease costs for Value Based Care (VBC) members. Helps lead HCC strategy and performance. Helps lead VBC quality efforts as well as participate in IU Health ambulatory quality efforts. Works with the IU Health physician enterprise to drive VBC priorities and efforts. Creates and drives post-acute strategies including SNF and readmission prevention strategies. Assists with creation and operationalizing new models of care such as hospital at home, primary care at home, connected care, etc.. Serves as physician consultant to Population Health care management and works with IU Health Integrated Care Management to coordinate care across the continuum.  Serves as consultant to and helps direct priorities and tactics for Disease Management program. Works closely with IU Health Plan leadership to achieve IU Health Plan priorities. Provides physician leadership over contracts with third party payers to assist with achieving success in these value-based programs. Assists with development and optimization of stratification algorithms and other Cerner based enhancements to achieve population health goals. Participates in monthly population health meetings with regional physician group and serves as a resource to assist them in their population health activities. 

  • M.D. degree is required. 
  • Must be a physician in good standing with privileges in an IU Health physician group. 
  • Requires at least 10 years of experience in healthcare administration with progressively increasing levels of responsibility. 
  • Requires experience with population health programs such as Quality, HCC performance, post-acute services, etc. 
  • Must be available for a 0.5 full-time equivalent (FTE) role and have the ability to consistently balance clinical practice commitments with those of this key population health role. 
  • Requires the ability to work with individuals at multiple levels within and external to the organization, work successfully across a highly matrixed organization and be a team player.
