The Bachelor of Social Work (BSW) is responsible for providing and coordinating behavioral health services for patients as part of an integrative model of care. The BSW collaborates with and assists  behavioral health staff in order to effectively manage mental health conditions and support physical health outcomes. The BSW helps coordinate behavioral health referrals, completes patient intakes,  educates patients about mental health and substance use conditions and treatment options, and monitors symptoms and response to services. These services are conducted in individual sessions and group sessions. Under the direct supervision of the Therapy Supervisor, this position supports the clinical care team, including LCSWs, MSWs, Clinic Managers and Primary Care Providers and other staff. Independent judgment is required to plan, prioritize and organize diversified caseload with professionalism and discretion. The position continually requires demonstrated poise, tact and diplomacy in working with a diverse group of internal and external partners. The BSW supports the organization's mission, vision and values. This position strives to be efficient, effective and collaborative and to demonstrate professionalism and accountability, while following departmental and clinic policies, procedures and protocols. The hours would be primarily days 8am-4:30 pm as needed and shifts may vary.

 0-3 years of experience required. Bachelor's Degree in Social Work required.
• Permanent LSW licensure preferred.
• Requires strong clinical assessment, planning and goal development skills.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Requires strong counseling skills to help patients and families understand, accept and follow medical recommendations.
• Requires proficiency in Microsoft Excel and Word.
