The Behavioral Health Specialist is responsible for tracking and managing patient registry and progress, tracking time devoted to behavioral health care provided, resource and referral management, support for integrated behavioral health in person and via phone, support patient staffing meetings, and related duties by working collaboratively with the integrated behavioral health team to achieve desired clinical service and cost outcomes. Interacts directly with patients to support interventions initiated by the behavioral health and primary care teams, assisting in screening and treatment response measurements, providing information on internal and external resources, and coordinating referrals for sites not physically staffed by the integrated care service. Also responsible for maintaining a registry of all patients interacting with the behavioral health team and tracking time spent on the care of each patient. Identify and triage crisis needs.
The specialist will support the resource and referral management for integrated behavioral health by working collaboratively with the integrated behavioral health team to achieve desired clinical service and cost outcomes. Interacts directly with patients and primary care staff to support interventions initiated by the behavioral health and primary care teams, including urgent behavioral health consults and collaborative care. The specialist will provide information on internal and external resources, and coordinate referrals to outside agencies. Cerner registration and scheduling training will be required upon hiring. Virtual position based out of Sleep lab, Indianapolis. Hybrid work from home and in PCP offices.
Hours: At this time, hours are 8:30-5, M-F. We do have plans to expand hours and some evenings to 7PM and an occasional Saturday will be likely in the future.
Experience: Experience in health care or mental health setting. Registration and case management experience is a plus.
