Job Description

Overview

The Director, Community Health, Social Determinants of Health (SDOH) will be responsible for developing and successfully implementing the strategy, operational work plan, and key activities for addressing social determinants of health for patients across the IU Health system (all hospital entities/regions) as well as to those in larger community and throughout state of Indiana.  This includes devising a successful strategy, as well as building an infrastructure with aligned key actions/plans and metrics to achieve success.  Key responsibilities will include:

  • Launch, expand and successfully implement a SDOH screening and referral process throughout IUH and the community
  • Oversee the strategy and operations for Integrated Social Work Hub
  • Devise & successfully implement strategy and operational plan for the following services: Find Help Now, Medical Legal Partnerships, and other key initiatives
  • Prioritize, coordinate, align, develop metrics and evaluate all activities related to SDOH across the system and in the communities serve

The leader will be responsible for developing and expanding services that impact social determinants of health, growing utilization of screenings and services, and analyzing operational data and metrics to make recommendations for improvement and advancement.  The leader will be responsible for developing, monitoring, and maintaining relevant internal and community partnerships that align with the work of social determinants of health to advance impact on the patients and communities served. The leader will play a key leadership role on the newly forming social determinants of health steering committee which will guide the work across the system.

The leader will also work alongside other Community Health department directors, health equity leaders, operational partners, various IU Health department leaders, and external stakeholders in a collaborative format to accomplish the work and achieve division goals.   The leader will be able to effectively lead strategy and operational excellence with influence across the IU Health system and greater community to drive change, alignment, and system and community impact.  Other related duties will be assigned as needed.

The leader will report to the VP, Community Health Operations and will have project management support to lead the work.  The leader will also work alongside other Community Health department directors, operational partners, health equity leaders, various IU Health department leaders, and external stakeholders in a collaborative format to accomplish the work and achieve division goals.   The leader will be able to effectively lead strategy and operational excellence with influence across the IU Health system and greater community to drive change, alignment, and system and community impact.  Other related duties will be assigned as needed.

Through its 16 hospitals (a mix of urban, suburban and rural facilities), IU Health offers a full range of specialty and primary care services for children and adults.  Community Health is an integral part of the System Health Solutions (SHS) division within Indiana University Heath.  The Community Health division improves the health and health equity of IU Health patients, communities, and the state of Indiana. SHS was formed in 2015 and is charged with making high quality care more accessible and affordable, creating a more seamless customer experience, and developing new care models and capabilities that improve health of our patients and communities

QUALIFICATIONS

  • Bachelor’s degree required; Master’s degree preferred
  • 7 - 10 years of experience in healthcare strategy, operations, business and/or health programming
  • Experience in healthcare leadership or relevant leadership experience preferred
  • Knowledge or experience in public health and/or social determinants of health preferred
  • Must be equally skilled at strategy development and operations – with a proven track record of managing and leading teams focused on strategy, execution, process improvement, and goal attainment
  • Skilled at data analysis and utilization of data and trends to inform decision making
  • Able to build and monitor successful relationships, coalitions and partnerships with key internal and community stakeholders
  • Must have demonstrated experience of delivering results in a matrix environment
  • Must demonstrate strong skills in organization, communication, and results driven leadership
  • Able to balance and deliver results in an environment with multiple competing priorities
  • Able to inspire teams, set a vision, drive outcomes and do so through influence (versus authority or direct reporting lines)
