Build your career with IU Health. We have an outstanding position open as a Quality Performance Improvement and Population Health Manager. This position is a Hybrid role with occasional visits to the office at our Gateway Plaza location in Indianapolis.
The Manager manages strategic quality and population health initiatives; leads clinical workflow evaluation and redesign to support best practices and workflow efficiencies that support quality of care and population health documentation, patient engagement, and metrics capture. Leads the technical development of quality and population health tools and develops education and communication strategies to keep Quality and Population Health teams, ambulatory providers and support staff abreast of new tools and opportunities for improvement. Provides leadership, training and professional development opportunities to team members of the Quality & Population Health departments; works closely with statewide outpatient quality and Population Health teams, physician practices and service line leaders to implement best practices for clinical workflow with goal of improving quality of care at point of service.
