The Tobacco Treatment coach is primarily responsible for delivering tobacco treatment intervention utilizing health coaching skills of motivational interviewing, stages of change theory, and appreciative inquiry. Engages risk stratified members by providing health coaching sessions and onsite delivery of programs. Utilizes established protocols and collaboration with population health team members to support participants and create customized wellness plans.Preferred candidates have a strong background in tobacco treatment, behavioral health, and addiction and recovery treatment.
