The Tobacco Treatment coach is primarily responsible for delivering tobacco treatment intervention utilizing health coaching skills of motivational interviewing, stages of change theory, and appreciative inquiry. Engages risk stratified members by providing health coaching sessions and onsite delivery of programs. Utilizes established protocols and collaboration with population health team members to support participants and create customized wellness plans.Preferred candidates have a strong background in tobacco treatment, behavioral health, and addiction and recovery treatment. Preferred Tobacco Trained Specialist.

• Bachelor's degree in Nursing, Health Promotion, Health Education, or a health-related field required.
• Requires 3-5 years of health coach, wellness or other applicable experience.
• Health/Wellness Coaching Certification strongly preferred.
• Requires proficiency with Microsoft applications.
• Travel within the state required, minimal if any.
• Experience with Telephonic Health Coaching preferred
• Ability to be flexible in the workplace.
• Strong communication and collaborations skills are essential.
• Previous EMR or Cerner experience is preferred.
